Let’s be honest, location matters more than GPs realise!

When GPs first reach out to us, the conversation almost always starts the same way. They want to work in Sydney. Or Melbourne. Or maybe Brisbane. And that is completely understandable. These are world-class cities with incredible lifestyle, culture, and infrastructure.

But here is what our GP recruitment experts gently have to explain: for most international medical graduates (IMGs) and overseas-trained GPs, those big city dreams come with a significant catch when it comes to Medicare eligibility and billing rights.

This is why understanding the MMM classification system and DPA locations is not just a bureaucratic checkbox. It is honestly one of the most important things you need to get right before you even think about which GP roles in Australia you are applying for. Get this wrong and you could find yourself working in a location, which affects everything from your income to your long-term career pathway.

So let us walk through this together, the way we explain it to every GP we work with.

What is MMM1 in Australia?

Well, MMM stands for Modified Monash Model. It is a geographic classification system used by the Australian Government to categorise locations by their remoteness and access to healthcare services.

The scale runs from MMM1 through to MMM7. MMM1 is at one end of the scale and refers to major cities. Think Sydney, Melbourne, Brisbane, Perth, and Canberra. These are Australia’s largest urban centres with the highest population density and, crucially, the greatest existing access to healthcare services.

From a government policy perspective, MMM1 areas are considered well-served. There is no perceived shortage of GPs in these locations from a policy standpoint. For most IMGs, working in an MMM1 location means you are not eligible to access a Medicare provider number accessing Medicare rebates for a minimum of 10 years. This is legally known as the 10-year moratorium. And without a Medicare provider number, you cannot independently bill patients for consultations. This is a fundamental issue that affects your entire earning structure and employment model as a GP.

What is DPA in Australia?

DPA stands for Distribution Priority Area. This is a separate classification system used by the Department of Health to identify locations across Australia that have a genuine shortage of GPs and primary healthcare services.

DPA locations are areas where the government has determined that there is an unmet need for GP services. When you work in a DPA-designated location, as an overseas-trained GP you become eligible to apply for a Medicare provider number. This is the key that unlocks your ability to practice independently and bill patients through Medicare.

Most DPA locations sit in regional, rural, and outer metropolitan areas. Some outer suburbs of major cities actually hold DPA status, which surprises a lot of GPs when they first hear it. But broadly speaking, the further you move away from the dense inner-city cores of Australia’s major cities, the more likely a location is to carry DPA status.

What is the Difference Between MMM1 and DPA Location?

Think of it this way. MMM is about geography and remoteness. DPA is about GP workforce need.

A location can be in an outer suburb of a major city (technically still classified as MMM1 or MMM2) but still hold DPA status because there are not enough GPs serving that community. Conversely, a regional town might have a strong existing GP workforce and therefore not carry DPA status.

For GPs navigating opportunities in Australia, the DPA status of a location is arguably the more important of the two factors. It is the one that directly determines your Medicare eligibility as an IMG.

MMM1 locations generally do not carry DPA status. That is the practical reality for most big-city locations in Australia.

How MMM1 and DPA Impacts GP Jobs and Earnings in Australia

Here is the real-world impact, and this is where we always encourage doctors to pay very close attention.

As mentioned above, if you are an overseas-trained GP without fellowship (non-VR GP), working in a non-DPA location means you will likely be employed under a 19AB restriction, which prevents you from accessing a Medicare provider number in that location. Some practices will employ you under a different billing arrangement, but your income potential and independence are significantly limited.

In a DPA location, you can apply for a Medicare provider number, practice more independently, and access the full earning potential that comes with GP work in Australia. Many GPs working in DPA-designated regional and outer-metro areas earn very competitive packages, often higher than their counterparts in inner-city clinics, precisely because the demand for GPs is strong and practices are motivated to attract and retain good doctors.

GP recruitment in Australia for IMGs is heavily concentrated in DPA locations for this reason. It is not just where the jobs are. It is where the right jobs are for your qualification and expertise.

How is Lifestyle in MMM1 Big Cities vs DPA Regional Locations?

Now let us talk lifestyle, because this matters just as much as the clinical and financial picture.

Life in MMM1 Cities

Sydney, Melbourne, and Brisbane offer everything you would expect from world-class cities. Incredible food scenes, diverse communities, excellent schools, international airports, arts and culture. If you have a family and value access to a broad range of amenities, MMM1 cities are genuinely wonderful places to live.

The trade-off? These cities are expensive. Housing costs are among the highest in the world in some inner suburbs. Traffic and commute times can be significant. And from a GP work perspective, city-based clinics are often highly competitive environments with busy patient loads focused on high throughput rather than relationship-based care.

Life in DPA Regional and Outer Metro Locations

Regional and outer suburban Australia offers a genuinely different lifestyle experience, and for many international GPs, it turns out to be far more appealing than they initially expected.

Think lower cost of living, more space, shorter commutes, strong community connections, and a style of general practice that tends to be more holistic and relationship-based. Many regional locations offer outstanding natural environments, whether that is coastal towns in Queensland or New South Wales, the wine regions of South Australia, or the lush countryside of Victoria.

Number of patients in DPA locations is genuinely high, which means you are valued and needed. That feeling of making a real difference in an underserved community is something many of our placed GPs describe as one of the most rewarding aspects of their Australian career. The reality is that DPA locations in Australia offer some of the best GP opportunities in Australia right now, both financially and professionally, and many GPs who were initially hesitant about going regional end up absolutely loving it.

Make the Right Location Choice with Ray Rekruiters

FAQs

Can I work in Sydney as an IMG GP?

Yes, you can work in Sydney as an IMG GP, but there is an important limitation you need to understand before you make any plans around it. Sydney is classified as an MMM1 location, which means most areas within the city do not hold DPA status. Without DPA status, you will not be eligible for a Medicare provider number under the standard 19AB rules that apply to overseas-trained doctors.

The good news is that some outer suburbs around Greater Sydney do carry DPA status. These locations let you access a Medicare provider number and build your Australian career on solid financial footing. At Ray Rekruiters, we can help you identify which specific Sydney-adjacent locations are DPA-approved so you can make an informed decision.

How long do I have to work in a DPA location before I can move to a city?

This is one of the most frequently asked questions we hear from GPs planning their Australian career, and it is a great one to get clear on early. Under the current 19AB framework, non-VR overseas-trained GPs are generally required to work in a DPA location for a set period before they can access a Medicare provider number in a non-DPA area.

The timeframes and specific conditions can vary depending on your individual circumstances, your visa status, and any changes to government policy. As a general guide, doctors are often looking at a commitment of several years in a DPA location, but the exact requirement for your situation depends on your specific registration and employment pathway.

Do MMM1 areas pay more?

This is a really common assumption, and the honest answer might surprise you. MMM1 city-based roles do not necessarily pay more for IMG GPs. In fact, many regional and DPA-based GP roles offer equal or higher earnings than equivalent roles in big cities.

Here is why. In a DPA location, you have access to a Medicare provider number, which means you can bill patients directly and earn through a percentage of billings. City-based MMM1 clinics often employ IMG GPs under more restrictive arrangements that cap earning potential. On top of that, the cost of living in Sydney or Melbourne is significantly higher, so even if gross pay looks similar, your take-home quality of life is often better in a regional DPA role.

The bottom line is that DPA locations in Australia consistently offer some of the most financially rewarding GP opportunities available to international doctors.

Is visa sponsorship available for GP roles in DPA locations?

Yes, visa sponsorship is widely available for GP roles in DPA locations, and this is one of the genuine advantages of targeting DPA-designated practices when you are planning your move to Australia.

The most common visa pathway for overseas-trained GPs is the Skills in Demand visa, known as the subclass 482. Many DPA clinics and regional healthcare employers are approved sponsors and actively use this visa to recruit international GPs. The 482 visa typically allows you to work in Australia for two to four years initially, with the potential to transition to permanent residency through an employer nomination pathway such as the subclass 186 visa.

At Ray Rekruiters, we work with GPs from initial visa enquiry right through to placement, and we can connect you with the right visa assistance to ensure your visa pathway is handled correctly from day one.

An insightful read by Australia GP Recruitment Experts at Ray Rekruiters

If you are a GP from the UK, Ireland, or anywhere else in the world and you are seriously thinking about working in Australia, one of the first real questions you will have is: how much will I actually earn?

Well, Australia is one of the strongest destinations in the world for GP careers right now. A research report by the Australian Medical Association estimates that there will be an undersupply of around 10,600 GP FTEs by 2031–32. Clearly, the demand for general practitioners is high, the lifestyle is genuinely enviable, and the earning potential is significant. But your actual income will depend on a few key things: the billing model at your clinic, where in Australia you are working, and how experienced you are. Let us walk through all of that together.

How Does the GP Payment System Works in Australia?

To understand how GPs earn in Australia, you first need to understand Medicare. Medicare is Australia’s publicly funded healthcare system. It covers a wide range of medical consultations and services, and it is the foundation of how most GP income flows.

Here is what it really means in practice. When a patient sees a GP in Australia, Medicare assigns a fee to that consultation. This fee is called the Medicare Benefits Schedule (MBS) item number. The government pays a portion of that fee directly. The clinic may or may not charge the patient an additional amount on top of that.

Now here is the key difference from what many UK or Irish doctors are used to. Most GPs in Australia are not salaried employees in the traditional sense. They do not receive a fixed monthly salary the way you might in an NHS trust or an Irish public health setting. Instead, the vast majority of GPs earn income based on the number of consultations they do and the billing model the clinic operates under.

This is sometimes referred to as a percentage-based or fee-for-service model. The more patients you see and the more you bill, the more you earn. It is a different mindset, but once you understand it, it actually gives you a lot of control over your income.

3 Key GP Payment Models in Australia

1. Mixed Billing Model (Most Common)

This is the model you will encounter most frequently across Australian GP clinics. In a mixed billing setup, the clinic bulk bills some patients and privately bills others.

When a patient is privately billed, the clinic charges a fee that is higher than the Medicare scheduled fee. The patient pays the difference, which is called a gap payment. As the GP, you earn a percentage of the total billing you generate. This percentage typically ranges from 60 to 70 percent, though it varies between clinics and experience levels.

Many doctors from the UK are pleasantly surprised by this model because it directly rewards your work. The more patients you see and the more thoroughly you consult, the more you earn. It creates a strong link between your clinical effort and your financial outcome.

2. Bulk Billing Model

In a bulk billing clinic, the practice agrees to accept the Medicare benefit as full payment for every consultation. There is no gap charge to the patient. This makes healthcare free at the point of service for patients, which is great for accessibility.

From an income perspective, bulk billing tends to generate a slightly lower per-consultation fee compared to private billing. However, bulk billing clinics often see higher patient volumes because the service is free to patients. For GPs who are fast, efficient, and comfortable seeing high numbers of patients, this model can still produce strong earnings. Bulk billing is particularly common in outer suburban and lower-income areas, and in clinics that serve families, pensioners, and concession cardholders heavily.

3. Salary-Based GP Model

Some GPs in Australia are employed on a fixed salary basis. This is more common in community health centres, corporate healthcare groups, and certain hospital-based settings.

Salaried roles offer stability and predictability, which many GPs value especially when they first arrive in Australia. You know exactly what you will earn each month, which can make the transition and settlement process easier.

The trade-off is that you may earn less than you would in a percentage-based mixed billing role if you are a high-volume GP. However, salaried roles often come with additional benefits such as superannuation contributions (Australia’s pension equivalent), leave entitlements, and indemnity coverage included in the package.

What Does Actually Affect Your GP Income in Australia?

Understanding the models is one part of the picture. But let us also talk about what will actually shape your income day to day.

Location matters: GPs working in rural and regional Australia, particularly in areas classified as Distribution Priority Areas (DPA), often earn significantly more. These areas face serious doctor shortages, and clinics in these locations frequently offer higher percentage splits, relocation packages, and signing bonuses to attract GPs. If you are open to regional work, even for a period, your earning potential increases considerably.

Experience and fellowship status: A Vocationally Registered (VR) GP in Australia generally earns more per MBS item than a non-VR GP. This is because VR GPs can access a wider range of MBS item numbers, including longer consultations, chronic disease management plans, and mental health care plans. If you are not yet VR when you arrive, there are pathways to achieve it, and the income difference makes it well worth pursuing.

Billing structure and negotiation: The percentage you negotiate with your clinic directly impacts your earnings. This is where having the right recruiter in your corner makes a real difference. Knowing what is reasonable to expect and how to position your experience gives you a stronger starting point.

Patient volume and consultation length: In a fee-for-service model, volume matters. A GP seeing 30 to 35 patients per day in a busy mixed billing clinic can earn a very competitive income. As you build your patient base and your efficiency grows, so does your earning potential.

CLICK HERE to explore new GP opportunities with different billing model options across Australia.

What Can You Realistically Expect to Earn as a GP?

While individual incomes vary widely, here is a general sense of the range for context. A full-time GP in a metropolitan mixed billing clinic typically earns between AUD 250,000 and AUD 350,000 gross per year. GPs in rural or DPA areas, or those in high-volume private billing settings, can earn considerably more. Newly arrived GPs on salary or in lower-volume settings may start at AUD 180,000 to AUD 220,000 as they settle in, with income growing quickly as their patient base builds.

These are not guarantees. But they reflect real market conditions for GPs working in Australia today.

Your GP Career in Australia Is a Genuine Opportunity

The Australian GP landscape is genuinely exciting right now. There is a real shortage of general practitioners across metro, regional, and rural Australia. That means opportunities are plentiful, clinics are motivated to attract good doctors, and negotiating a strong package is very much possible.

If you are coming from the NHS or the Irish health system, the shift to a fee-for-service model can feel unfamiliar at first. But most GPs who make the move find that it rewards their commitment and clinical skills in a way that fixed salary structures simply do not.

The key is understanding how the system works before you arrive, so you can make informed decisions about where to work, what model suits you, and how to set yourself up for success from day one.

FAQs- What GP Doctors Most Often Ask

How much does a GP earn in Australia?

A full-time GP in Australia typically earns between AUD 250,000 and AUD 350,000 per year in a metropolitan mixed billing clinic. GPs in rural or DPA areas can earn significantly more, often exceeding AUD 400,000 annually.

Are GPs in Australia paid a salary or per consultation?

Most GPs in Australia are not salaried. They earn income through a fee-for-service model, receiving a percentage of the Medicare and private billing they generate per consultation.

What is the most common GP payment model in Australia?

The mixed billing model is the most common. GPs bulk bill some patients (fully covered by Medicare) and privately bill others, earning a percentage of total consultations billed, typically 60 to 70 percent.

What is bulk billing in Australia?

Bulk billing is when a GP accepts the Medicare Benefits Schedule (MBS) fee as full payment for a consultation, with no out-of-pocket cost to the patient. The government pays the clinic directly.

Do GPs in rural Australia earn more?

Yes. GPs working in Distribution Priority Areas (DPA) or rural and remote regions typically earn more due to higher percentage splits, relocation incentives, and signing bonuses offered by clinics facing doctor shortages.

Expanding Opportunities for General Registration GPs in Australia:

Breaking Down 19AA Legislative Restrictions

Are you a General Registration GP in Australia who has completed the RACGP PEP via the Standard Pathway and holds Australian Citizenship or Permanent Residency? If so, you’re in luck! There’s a pathway that exempts you from certain legislative restrictions, significantly expanding your work options.

Let’s dive into the details:

Understanding 19AA Legislative Restrictions

Firstly, it’s essential to grasp the context of the 19AA legislative restrictions. These regulations limit where certain medical practitioners, including General Registration GPs, can work based on factors like location and visa status. For many, these restrictions have posed challenges in finding suitable employment opportunities, particularly in metropolitan areas.

The PEP Advantage

However, if you’re on the path to obtaining fellowship with RACGP through the Practice Experience Program (PEP), you’re in a unique position. Completing the PEP via the Standard Pathway grants you exemptions from some of these restrictions, opening up a world of possibilities.

Expanded Work Opportunities

Under this exemption, you’re eligible to work in Approved Medical Deputising Services (AMDS) and Aboriginal Community Controlled Health Services (ACCHS) clinics in Modified Monash Model (MMM) 1 locations. These clinics do not need to be classified as District of Workforce Shortage (DWS) for that area, as Aboriginal health services automatically receive a DWS status.

Breaking Down the Requirements

For AMDS roles, there’s a slight additional requirement. You’ll also need to work concurrently at a daytime clinic in MMM 2-7 locations. This condition ensures that healthcare services are provided across various settings, benefiting both practitioners and patients.

Simplifying the Process

To take advantage of these expanded opportunities, all you need to do is apply for a change in circumstances via your PEP portal. The RACGP Approved Placement PEP team will conduct a full review for practice and placement approval, streamlining the process for you.

The Metropolitan Advantage

This exemption paves the way for General Registration GPs to work in metro areas closer to the city, whereo there’s often high demand for healthcare services. By removing barriers to entry, more practitioners can contribute their skills and expertise to meet the needs of diverse communities.

The exemption from 19AA legislative restrictions for General Registration GPs completing the RACGP PEP via the Standard Pathway is a game-changer. It not only expands job opportunities but also fosters a more inclusive and accessible healthcare system across Australia. So if you’re on this journey, rest assured that the doors are wide open for you to make a meaningful impact in the medical field.

The Impact of AMA’s Bulk Billing Incentives on GPs and Patients in Australia

In a landmark move, the Australian Medical Association (AMA) has implemented game-changing bulk billing incentives that promise to reshape the landscape of healthcare in the country.

Affordable Healthcare Access

The cornerstone of AMA’s bulk billing incentives lies in making healthcare services more accessible to a wider population. With direct billing to Medicare, patients can now receive medical attention without the looming burden of out-of-pocket expenses. This shift addresses a fundamental challenge in the Australian healthcare system—ensuring that quality healthcare is available to all, irrespective of financial constraints.

Elevating Preventative Care

One of the key advantages of bulk billing incentives is their potential to encourage a proactive approach to healthcare. GPs are empowered to focus on preventive care measures, conducting regular check-ups and screenings that contribute to the early detection and management of health issues. This shift from reactive to preventive care not only improves patient outcomes but also significantly reduces the strain on the healthcare system.

Financial Relief for Patients

The financial strain associated with healthcare is a major concern for many Australians. AMA’s bulk billing incentives alleviate this stress by removing financial barriers to medical care. Patients, especially those managing chronic conditions, can now access essential healthcare services without worrying about the financial toll. This not only improves the overall health of individuals but also promotes a healthier and more resilient society.

Sustaining GP Practices

The implementation of bulk billing incentives is a strategic move to sustain GP practices across the nation. By streamlining billing processes and ensuring prompt reimbursement, the AMA is supporting the financial stability of GP practices. This, in turn, allows GPs to maintain high-quality standards of care and invest in the continuous improvement of their services.

Strengthening Doctor-Patient Relationships

Removing financial barriers has a profound impact on the dynamics of doctor-patient relationships. With bulk billing incentives in place, GPs can dedicate more time to patient care, fostering open communication and trust. Patients feel more comfortable discussing their health concerns, leading to a more holistic and patient-centric approach to healthcare.

How This Benefits Job Seekers in Australia

These bulk billing incentives create a more attractive and sustainable environment. The demand for skilled healthcare professionals is likely to rise as accessibility improves, preventive care becomes a priority, and GP practices thrive. As the healthcare system evolves, job seekers can find exciting opportunities to contribute to a more inclusive and efficient healthcare ecosystem. In essence, the AMA’s bulk billing incentives not only elevate the quality of healthcare but also open up new horizons for those aspiring to make a difference in the Australian healthcare landscape. The future of healthcare is brighter, more accessible, and full of opportunities for both GPs and individuals looking to embark on a fulfilling career in the field.

What is the Health Workforce Certificate (HWC) and why was it required for IMGs in the past?

The Health Workforce Certificate (HWC) was a mandatory requirement for International Medical Graduates (IMGs) seeking employment in the Australian healthcare sector. It aimed to ensure that IMGs met specific criteria before they could practice medicine in Australia. The certificate served as a means to regulate and maintain the quality of healthcare professionals in the country.

What is the recent policy change regarding the HWC?

The Australian Department of Home Affairs has recently abolished the requirement for IMGs to obtain a Health Workforce Certificate (HWC) when applying for healthcare jobs in Australia. This policy shift simplifies the application process for IMGs and offers them a more streamlined pathway to practice medicine in Australia.

How does this policy change benefit IMGs?

This policy change benefits IMGs in several ways: Faster Entry: IMGs can apply for jobs in Australia without the added burden of acquiring an HWC, expediting their entry into the workforce. Career Advancement: IMGs can advance in their careers more swiftly since the HWC requirement no longer applies, streamlining the application process. Diverse Expertise: Australia gains access to a more diverse range of medical expertise, ensuring a broader spectrum of healthcare services for its population.

What should IMGs consider during this transformative phase of the application process?

IMGs should consider partnering with trusted experts to navigate the complexities of the immigration process and the Australian healthcare sector. Ray Rekruiters, a specialist in healthcare workforce solutions can guide IMGs through a seamless immigration journey, ensuring a smooth transition into the Australian healthcare system.

How can IMGs stay updated on developments in the world of international medical graduates and healthcare in Australia?

IMGs can stay informed by regularly checking official government websites, healthcare organizations, and trusted sources in the medical field. Additionally, you can follow reputable news outlets and organizations specializing in healthcare workforce solutions like Ray Rekruiters for the latest updates and guidance.

This FAQ guide provides valuable insights into the recent policy change and offers advice on how to make the most of this opportunity. Remember to stay informed and consider seeking expert assistance to ensure a smooth transition into your medical career in Australia.
